Once you have selected a dentist, or if you have any questions about your selection, please contact our Member Services Department at 888-703-6999 between the hours of 8:00 am to 5:00 pm Monday through Friday.

How do I make an appointment?
After completing your enrollment form and selecting your Primary Care Dentist, you are eligible to receive care on the first of the month following LIBERTY's receipt of your enrollment application and premium.

Please note that you must select a Primary Care Dentist and submit your selection to LIBERTY before you call to make an appointment. You may call your selected dentist at any time after the effective date of your coverage, typically thefirst of the month following receipt of you application and premium.

Be sure to identify yourself as a member of LIBERTY Dental Plan when you call the dentist for an appointment.
