How do I receive care?
You must choose a Primary Care dentist when you enroll in the plan.

You are free to choose any Primary Care Dentist off of our directory. You and your entire family must use the same dentist. Once you have selected a dentist, or if you have any questions about your selection, please contact our Member Services Department at 888-703-6999 between the hours of 8:00 am to 5:00 pm Monday through Friday.

How do I make an appointment?
After completing your enrollment form and selecting your Primary Care Dentist, you are eligible to receive care on the first of the month following LIBERTY's receipt of your enrollment application and premium.

Please note that you must select a Primary Care Dentist and submit your selection to LIBERTY before you call to make an appointment. You may call your selected dentist at any time after the effective date of your coverage, typically the first of the month following receipt of you application and premium.

Be sure to identify yourself as a member of LIBERTY Dental Plan when you call the dentist for an appointment. We also suggest that you keep you Evidence of Coverage and Schedule of Benefits handy when you go to your appointment. You can then reference benefits and applicable co-payments which are the out-of-pocket costs associated with your plan.

What if I have questions about my dental plan?
Call LIBERTY Dental Plan's Member Services department Monday through Friday 8:00 a.m. to 5:00 p.m. at 888-703-6999. We are here to assist members with questions about their dental benefits.

The hearing and speech impaired may use the California Relay Service's toll-free telephone numbers: 1-800-735-2929 (TTY) or 1-888-877-5378 (TTY) to contact the department.
